OUR PROCESS
HOME ASSESSMENT
The initial in-home assessment allows you to tell us what you need. You then have the opportunity to ask for input from our experienced home care professionals, who are happy to guide you to find the best solution for your situation. Countryside Care considers your interests, passions, likes, and dislikes to create a customized care plan that meets the needs of you and your loved ones. During the initial in-home assessment, we will discuss the client care contract, price, and answer any outstanding questions.
CAREGIVER SELECTION
Our screening process is very thorough to ensure all of our caregivers are a good match with our company. We perform a vulnerable sector criminal record check, reference checks, and we require specific certifications based on the position. We then consider you and your loved one’s interests, passions, likes, and dislikes to ensure the selected caregiver is the right fit for you and your customized care plan.
FIRST VISIT
During the first visit, you will be introduced to your selected caregiver. Accompanied by an experienced Countryside Care representative, your caregiver will complete your customized care plan. We take this opportunity to make adjustments to your care plan and solidify your care plan moving forward.
SECOND VISIT
Your selected caregiver will arrive independently for your scheduled visit.
FOLLOW-UP & FEEDBACK
Countryside Care is here when you need us, twenty-four hours a day. Your feedback is important to us, and we encourage you to make adjustments to your care plan as needed. Any adjustments are taken care of according to your wishes.
MEET THE TEAM

Anna Hodgson
Co-Director
Anna is our Co-Director. She is responsible for coordinating the home health assessments of clients and developing care plans with our clients and families. Anna holds a Bachelor's Degree from Western University, and a Diploma in Project Management. In addition, Anna has earned a Business Certificate from Harvard Business School, specializing in Business Analytics, Economics for Managers, and Financial Accounting. Your health is in good hands with our expert staff.

Autumn Prestwich
Co-Director
Autumn is our Co-Director. She is responsible for leading and supervising all Countryside Care staff. Autumn manages the services provided by the Countryside Care team and ensures the delivery of high-quality client care. Autumn holds a Bachelor's Degree in Health Sciences from Western University. She completed her Bachelor of Nursing degree in 2018 and has experience working in acute and long-term care.
